Job Title: Nurse Practitioner – House Calls Locations available: Winter Park, Orlando, Lake Mary and Altamonte Schedule: Open and flexible Comp: 1099 Position Summary We are seeking a compassionate and highly skilled Nurse Practitioner (NP) to provide high-quality, patient-centered care in the home setting, with an emphasis on proactive care management, risk stratification, and care coordination across the continuum. This position is ideal for clinicians who thrive in an autonomous environment, are organized and comfortable working in a mobile environment. Key Responsibilities · Perform comprehensive History & Physical (H&P) assessments for new patient enrollment and risk adjustment · Conduct Transitional Care Management (TCM) visits within appropriate post-discharge timeframes to reduce readmissions · Complete Annual Wellness Visits (AWVs) and other preventive care services aligned with CMS guidelines · Perform in-depth functional, cognitive, and psychosocial assessments (ADLs/IADLs, fall risk, frailty, depression screening) · Provide ongoing primary care services, including diagnosis, treatment, and management of acute and chronic conditions · Develop and execute longitudinal care plans aimed at improving quality and patient outcomes · Provide early intervention to prevent unnecessary ED visits and hospital admissions · Collaborate closely with care coordinators and other interdisciplinary teams to coordinate care transitions across acute and post-acute settings. · Order and interpret diagnostic tests, labs, and imaging as appropriate · Prescribe medications appropriately, with attention to polypharmacy and deprescribing when indicated · Deliver patient and caregiver education to improve adherence and self-management · Maintain timely, accurate, and compliant documentation in the EMR · Participate in quality improvement initiatives and adhere to clinical protocols--- Performance Expectations (Value-Based Focus) · Work to reduce hospital admissions and 30-day readmissions · Decrease emergency department utilization · Achieve high completion rates for AWVs and preventive services · Support initiatives to maintain and improve quality metrics · Drive high levels of patient and caregiver satisfaction · Effectively manage a panel of patients Travel Requirements · Travel to patient homes within an assigned geographic territory · Optimize routing and scheduling to maximize efficiency and patient access · Flexibility to accommodate urgent visits for high-risk patients when needed or work with care coordinators to facilitate alternative care. Qualifications · Current, unrestricted Nurse Practitioner license in the state of practice · Board certification (AANP, ANCC, or equivalent) · Active DEA license (or ability to obtain) · Minimum 2+ years of experience as an APRN · Strong clinical assessment and diagnostic skills · Ability to work independently with sound clinical judgment · Excellent communication and interpersonal skills Requirements · Reliable transportation and valid driver’s license · Willingness and ability to travel within assigned service area · Proficiency with mobile EMR platforms and point-of-care documentation · Ability to work independently in a field-based environment · Ability to lift/transport basic medical equipment as needed Preferred Experience · Experience in home-based care, geriatrics, or value-based care models · Familiarity with Transitional Care Management and general primary care · Experience managing complex, high-risk patient populations Work Environment · Field-based role providing care in private homes, assisted living, and group home settings · Supported by a centralized care team, including care coordinators, and administrative staff · High degree of autonomy with strong clinical collaboration Compensation & Incentives · Pay per encounter · Travel fee per encounter · Laptop provided · Cell phone reimbursement
